Your health record

Woollahra General Practice keeps a record of your health information for the purpose of your care. That record includes your name, contact details, Medicare number, and the clinical notes, results and correspondence relating to your treatment.

Your record is held securely. It is available to the doctors and staff involved in your care, and is not disclosed to anyone else except where you have consented, or where the law requires it.

Seeing your record

You may request access to your health record, and you may ask us to correct anything in it that is wrong. Please speak to reception or to your doctor.

This website

This website is a set of static pages. It sets no cookies, runs no advertising, and collects nothing about you when you read it. It does not use analytics.

Two things on it are provided by other organisations, and using them takes you to them:

• Online booking is provided by AutoMed Systems. When you press Book online you leave this website and their privacy terms apply.
• Map links open Google Maps in a new tab if you choose to follow them. Nothing is loaded from Google unless you do.

The fonts and images on this site are served from this website itself, so reading a page contacts nobody but us.
